Assessment tasks
You are encouraged to carry out your own research using various resources such as the internet and books. Use Arial font size 11. Please note that your research should be used to support your essay. When addressing the task, write in a logical and structured way. Use of headings is permitted. Include a reference list (Harvard style) and also a Bibliography. Sources used should be a minimum of 10 each. All questions and answers should be arranged in one document, you may include photographs or DRAWINGS which are relevant to this subject. There is a guided word count on each of the task.
Write a report with the title:
ASSESSMENT AND CARE PLANNING
Make sure that within your report you:
Task 1 (650-700 words)
Describe a range of care planning assessment models and tools, contextualised to health and social care situations/examples.
Task 2 (650-700 words)
Evaluate care planning policies related to practice implementation and delivery; to include explanation of user defined outcomes, empowerment and wellbeing.
Task 3 (650-700 words)
Discussion of monitoring and review processes related to care planning, providing examples to support discussion.
Guidance
The first part is straight forward and although it asks for a description you must still provide evidence because you will be describing models and tools and relating these to health and social care contexts.
When evaluating care plan policy, you will need to draw on your own research and even experience if you have it.
When discussing monitoring and review processes make sure you include connections to underpinning values and principles which situate the needs of service users at the centre of all decisions.

These texts are not essential reading for the unit or course but merely examples of the types of supporting texts that are available if you wish to further your knowledge and understanding outside the scope of the course or for personal interest.
